Becoming a Collaborating Partner

We welcome groups to become a collaborating partner for the Japan Earthquake Relief Fund. Please contact the JASI office at 317-635-0123 or by email at admin@japanindiana.org so that we can officially include you in the effort.

Download this flyer and insert your organization name where indicated to use in your promotion of the Japan Earthquake Relief Fund.

Donation Registration Card (download here)
Download this card to get contact information from donors who would like their name to be included on the donor list and / or receive an acknowledgement for tax purposes. When your fundraising campaign is complete, please send the forms you have gathered to JASI and we will provide the acknowledgement to the donors.
